Early planning and preparation are essential for success in adulthood. Transition Individual Support Coordinators assist individuals, families, and schools in planning and coordinating services for life after graduation. For ages 14 through high school graduation, we link families with community resources, employment, and social activities each student wants.

Transition Collaborative

The Fairfield County Transition Collaborative was formed in 2009. This group of professionals meets twice a year to discuss local services, state initiatives, and networking of all agencies, schools, and families that work with transition youth. The Fairfield County Transition Collaborative serves as a model for the State of Ohio.

Fairfield Medical Center (FMC) Project SEARCH

This training model is a collaborative effort between the Fairfield Medical Center, Eastland-Fairfield Career & Technical Schools, Opportunities for Ohioans with Disabilities (OOD), and the Fairfield County Board of DD’s JobFusion Employment Service. This program is based at Fairfield Medical Center in Lancaster, Ohio throughout the academic school year and provides each participant with an opportunity to experience a variety of internships at the medical center. Program hours are Monday through Friday, 8 a.m. to 2:30 p.m.

The program works to improve and strengthen the student’s focus, increase independence, and learn transferable skills to obtain competitive employment. A special education instructor from Eastland-Fairfield Career & Technical School and job coaching services are on-site as well. Students need to meet high school graduation criteria and agree to attend one more year of educational training.
